Health Record Requirements

Early Childhood/Pre-Kindergarten

  • Physical Exam on Illinois State Form prior to the first day of school

  • Documented up-to-date Immunizations prior to the first day of school

Kindergarten (or first grade if first time in school)

  • Physical Exam on Illinois State Form prior to the first day of school

  • Documented up-to-date Immunizations prior to the first day of school

  • Eye Exam on Illinois State Form by Oct 17th

  • Dental Exam on Illinois State Form by May 15th

Second Grade

  • Dental Exam on Illinois State Form by May 15th

Sixth Grade

  • Physical Exam on Illinois State Form prior to the first day of school

  • Documented up-to-date Immunizations prior to the first day of school

  • Dental Exam on Illinois State Form by May 15th

 

All Grade Levels

  • Up-to-date Immunizations, Physical Exam on Illinois State Form prior to the first day of school

Transfer Students

  • Same as above per grade level within 30 days of registration

  • Students entering an Illinois school from out of state or out of country for the first time at any grade level must also provide the physical exam form.  The exam must be completed no earlier than a year (365 days) prior to the start of attendance at an Illinois school.

  • Student new to the Illinois School System will need an Eye Exam on the Illinois State Form within 30 days from registration

Medical Forms

Asthma Action Plan
The Asthma Action Plan is a written plan that is developed with the student's doctor to help control his/her asthma.  This plan gives the district important information about the student's daily treatment, such as what kinds of medicines have been prescribed and when they are taken.  The plan describes how to control asthma long term AND how to handle worsening asthma or attacks.  The plan explains when to call the doctor or go to the emergency room.  A completed form must be presented to the school office.  Click here to access a blank Asthma Action Plan that you may take to the doctor.

Food Allergies
Students with severe allergies should have a Food Allergy/Anaphylaxis emergency plan on file with the school nurse.  Click here for a Food Allergy Emergency Care Plan.  Please present the completed form to the school office.

Seizure Action Plan
Students prone to seizures should have a Seizure Action Plan on file with the school nurse.  Click here for a Seizure Action Plan.  Please present the completed form to the school office.

Concussion Plan Document
Beginning with the 2016-2017 school year, Illinois school districts are required to implement a research-based response system to concussion and head injuries.  Click here to access the Concussion Plan.

Dental Examination
All students entering Kindergarten, 2nd grade, or 6th grade must have a dental exam completed.  Click here to access the Dental Examination Form.

Religious Exemption Form
Parents or legal guardians who are requesting a religious Exemption to immunizations or examinations must use this form for students entering kindergarten, sixth, or ninth grades.  Click here to access the Religious Exemption Form.

Dental Safari 
Dental Safari's mobile dental office is a self-contained, fully-functioning dental operatory where children receive the same professional services as those performed in a traditional office.  Dental Safari visits Signal Hill School each Spring.  Click here to access the Dental Safari Permission Form.

Physical Examination
All students entering Kindergarten, 6th grade, or entering an Illinois school for the first time need to have a new physical exam and the required immunizations.  Questions about these requirements can be directed to the school office and/or the school nurse.  Click here to access a physical examination form.

Sports Physical Examination
A sports physical is required each year for those students in grades 5-8 who want to try out for a sport.  All students trying out must have a current physical on file BEFORE trying out.   Please note, sports physicals will NOT cover the physical grade level requirements for PreK, K, and 6th grades.  When in doubt, please use Physical Examination form (listed above) as it will cover both grade level AND sports requirements.  Click here to access a Sports physical examination form.

Vision Examination
All students entering Kindergarten need to have a vision examination before October 15 of their entrance year to school.  Click here to access the Eye Examination form.

Over-the-Counter Medication Authorization
Parents/Guardians who feel their child's discomfort level is preventing him/her from learning will need to complete this form if you would like for your child to have access to over-the-counter medication during school hours.  These medications would be given only to those students with minor complaints who are otherwise in good health and who have physician and parent authorization.  Click here to access the Over-the-Counter Medication Authorization form.

Prescription Medication Authorization Form
This form is required when a students need to bring prescription medication to be taken at school.  This form must be completed by both a parent/guardian and the child's doctor.  Click here for the Prescription Medication Authorization Form.
